NOTE: Patient Care Records will not be released unless a request is accompanied by a duly executed and complete HIPPA Compliant Patient Records Release Form signed by the patient and/or legal guardian. The District will only accept original forms. The District will not accept photocopies, facsimile transmissions or email transmissions of Patient release forms.

Note: Fire District maintains its records in compliance with all applicable guidelines, and records are destroyed consistent with the State of New Jersey Destruction of Public Records Act, Chapter 410, PL 1953. When records are beyond the required retention schedules, an application is made to the State and records are destroyed only after the State has approved their destruction.
